To process this application, you (the applicant’s supervisor, manager, or employer representative) must complete the form below and verify applicant’s identity. We suggest the applicant provide the required details (including passport style digital photo) to you with a link to this form for completion and payment. Please be aware, IBAC will no longer send Crew Cards via national postal services but will only ship via FedEx or DHL (with account number) or via FedEx Economy services for a USD 50 fee in North America. See Shipping Fees Table below for other regions. For shipments beyond those listed, we will contact you with shipment price after consulting with FedEx.

Shipping

Please include a FedEx or DHL account number for shipment of your Crew Card(s). If you do not have one, there will be a USD 30 charge for shipping via FedEx Economy in North America. For shipments beyond North America, we will contact you with shipment price after consulting with FedEx. IBAC will no longer send Crew Cards by national postal services due to significant delays and inability to track shipments.

Supervisor Information and Authorization
  • The above applicant is employed by, and performs flight crew duties for a Member Company of an IBAC Affiliated Association.
  • The submitted photograph is that of the applicant.
  • The Company assumes responsibility for the control and proper use of the Card and assumes all/any responsibility and liability arising from misuse of the Card.
  • The Card is valid for two (2) years from the date of issue so long as the applicant remains in the Company’s employ and the Company retains their affiliated association membership.
  • The Company will be responsible for retrieving the Card from exiting employees and for returning such Cards to IBAC for cancellation.
  • IBAC reserves the right to withdraw the Card at any time if, in its opinion, the Card is misused.
